Preferred name & Photo Request

Please fill out this Form with your preferred name & photo (can be your headshot or something more personal, keep it appropriate) for the Match Day Presentation that will be shown at the event. This form also confirms your attendance at the event.
- Deadline to submit: March 1^st, 2025

Additionally, Student Affairs staff is dedicated to improving the event experience for our students. To assist, we want to provide the opportunity for students to submit an audio file of the pronunciation of their name.
- This is not required. Only submit if you are concerned with the potential mispronunciation of your name.

"Day-of" Information

Parking:

Kansas City: Free parking is available on levels 5 & 6 of the Cherry St. Parking Garage at 5005 Oak St. from 9:30-12:30pm

St. Joseph: Free parking is available in the Mosaic Parking Garage, 4h or 5th floor.
